Q. What if I already have a reservation in VAMS?
A. If you would prefer to come to this drive-thru clinic, please register first, then cancel your appointment from within VAMS.
Q: What if I can’t attend this clinic?
A: You can still use the CDC’s VAMS system to schedule an appointment at another clinic. You can go to libertydoctors.com/vaccine to request an invitation. You can also click here to request notification if we schedule a new drive-thru clinic.
Q. Who can attend the clinic?
A. Liberty Doctors patients who qualify for Phase 1a (frontline healthcare workers and patients 65 years and older) may attend the clinic.
Q. Can I attend this drive-thru for my second dose?
A. Yes, if your first dose was Pfizer.
Q. Do I have to bring anything to the clinic?
A. If possible, please bring a photocopy of the front and back of your picture ID and insurance card (if you have a Medicare Advantage card, please include the Red White and Blue original Medicare card). If you are not able to make photocopies, please bring picture ID and insurance card(s). Also, please print and complete the pre-vaccination questionnaire and bring it with you on the day of the clinic. Click here to download the questionnaire.
Q. Will I receive an email confirmation?
A. Patients who enter an email address on the registration form will receive an email confirmation. If you do you not receive it, please check your spam folder. If you do not enter an email address, you will not receive a confirmation.
