Completing your paperwork ahead of time will expedite the vaccination process.
  • Print, complete and bring the new patient paperwork, encounter form, and vaccination questionnaire. CLICK HERE to access the forms.
  • Bring photocopies of the front and back of your picture ID and health insurance card.
  • Blank forms and photocopiers will be available for those who require them.
If this is your first dose, will return to the same location on Thursday, May 6th for your second dose, at the same time slot as your first appointment.
