Completing your paperwork ahead of time can expedite the vaccination process.
  • Print, complete and bring the Encounter Form and Prevaccination Questionnaire. CLICK HERE to access the forms.
  • Bring the gray COVID-19 vaccination card you received in with your first dose. If you have lost that card, we can issue you a new one.
*If you were an unscheduled vaccination, please arrive any time between 10am and 1pm.
