Please arrive at the same time slot as your appointment three weeks ago. If you cannot, or if were an unscheduled vaccination, you may arrive at any time between 9am and 1pm.
If you are unable attend this clinic, you can receive your second dose at any of our other clinics. They accept walk-ins. Info can be found at: libertydoctors.com/vaccine
Completing your paperwork ahead of time can expedite the vaccination process.
  • Print, complete and bring the Encounter Form and Prevaccination Questionnaire. CLICK HERE to access the forms.
  • Bring the gray COVID-19 vaccination card you received with your first dose. If you have lost that card, we can issue you a new one.
